Waiver

By booking a service, enrolling in training, or participating in any treatment with Amour Lashes, you acknowledge and agree to the following:

I understand that all services and trainings, including but not limited to lash lifts, lash extensions, brow laminations and teeth whitening treatments and related procedures, carry potential risks such as irritation, allergic reactions, or other unforeseen outcomes.

I voluntarily choose to receive services and/or participate in training and accept full responsibility for any risks, results, or outcomes that may occur.

I agree to release, discharge, and hold harmless Amour Lashes LLC, its owner, employees, and affiliates from any and all liability, claims, demands, damages, or causes of action arising out of or related to any services or training provided.

 

I acknowledge that results may vary and that no guarantees have been made regarding outcomes.I confirm that I have disclosed any relevant medical conditions, sensitivities, or allergies and understand that failure to do so may increase the risk of adverse reactions. Allergic reactions can result at any time with a service, and I acknowledge that I will seek medical assistance if needed, but know and acknowledge that Amour Lashes LLC is not responsible for any cost. 

 

By proceeding, I acknowledge that I have read, understood, and agreed to this waiver in full.
